Assessments & Intake
Welcome to the start your journey toward recovery with True North Recovery Inc.
Your first step is to complete the Walk In Assessment Application. This form must be completed prior to receiving an assessment or entering treatment at True North Recovery.
How Long Does it Take?
Please prepare to spend 20–45 minutes to complete the intake documents. If completed online, your wait time for walk in assessments and entering treatment will be drastically reduced.
The information gathered from these documents will help our team best assist you, so please be thorough and honest.

Sober Living Homes
The first step to being considered for our Sober Living Homes is to complete the True North Recovery ‘Sober Housing Program Application’. True North Recovery Inc. operates three recovery residences for both adult men and women. Our housing differs from traditional sober living because it is a program developed to help an individual gain skill in a supportive, structed environment so that one can successfully transition to independence in recovery.
How Long Does it Take?
Please plan on 15-20 minutes to completely fill out this form. it is a very important part of our application process and must be completed to proceed.
It is important to note that participation in True North Recovery’s Housing Program is contingent on engagement in treatment services. Therefore, you will also need to complete our intake paperwork once accepted and admitted into housing.
